Health care transition is the process of changing from a pediatric to an adult model of health care.  This is an important time for young adults as there are significant differences between adult and pediatric health care. 

The importance of supporting and facilitating the transition of adolescents with special health care needs into adulthood cannot be overstated.  

Here are some tips for adults and families to consider when looking for Healthcare for adults with disabilities:

  1. Include the young adult in discussions about their care 
  2. Identify a health care professional who can provide developmentally appropriate care and understands the unique challenges of transition
  3. Speak with your doctor about care coordination and future health care planning
  1. Maintain an up-to-date medical summary to share with health care professionals
  2. Ensure comprehensive and continuous health insurance is available during the transition from pediatric to adult care.
